Always Roland for a Fallout game, or in Oblivion, or any other RPG (with the exception of Pokemon).
Always Cptn. Cabbage for an FPS or shooter.
Always my own name for a Pokemon game.

I like using names from Anime and TV my friends don't watch so they think I'm original when coming up with cool sounding names but in reality I'm the most unoriginal person ever.

girl: Izumi, Eris,
Guy: Gourry, Zelgodis, Xanifer, Vrumugun

Mostly from Slayers. I've also used Neebs to name a Ranger my d&d group encountered when I was DMing

Tarkin. Used to use the last name of a girl I liked, somehow it lost a few letters, thought it was cool so I used it. It also is kind of ambiguous (I think that's how to use the word...), so it can be a female or male name. If I used the word ambiguous don't hold it against me, it's been a rough day

Other than that I try to make unique names, or if I want it to be like me I use my last name. Last names are cooler than any first in my opinion

For nearly every game I have played in the last 4 years, Axel. Up until then I'd been using Link (I was a HUGE Zelda geek back then). Although in Diablo 2 I have characters by the names of Almeth (Necro), Carmith(Barbarian), and Girnak (Druid)

Seif (m) / Acciador (f) is the main name i use then in no particular order Styrm, Draimen, Theron, Orestes, Malichi, Cyruz, and Darkrose.

In Eve online Topher the Gimp, I took my middle name and chopped it then added the descriptive gimp because I am in a wheelchair. :)

Those are the first names I usually use.

Sometimes, I'll get a wild hair up my ass and make up silly or dramatic names.
My good-guy in fallout 3/new vegas, I named Tobius Le'Saint. XD

My bad character, I'll probably choose a female and name her Sam Carnage,
or something to that effect.

Kathorias for males and Kaelissa/Lyrianna for females. I have a few other ones from being a bit of an altaholic in MMOs, but those are the main ones.
