Welp, you're a braver man/woman than I. I've read the plots of all the films mentioned (except for things like Requiem, which is disturbing in a whole different way) and I think I'm going to throw up. :(
I agree. I thought i had a pretty high threshold but Jesus. Wikied the plot of a serbian film, audition and August Underground's Mordum and i never want to see anything to do with those films again. No idea why people would watch that, they sound genuinely disgusting.

Bug comes to mind. It's not particularly gory (though there is a bit of blood and if you're disturbed by someone ripping out their own tooth then yeah.) To put it simply, It's about a mentally ill man who... Bah, let wikipedia spoil it for you if you want to do some research before watching it.

There was this one film that really disturbed me...
It wasnt some slasher movie or a psychlogical thriller,
in fact it was quite the opposite...it was laballed a ``comedy´´.
Try it if you dare, it certanly will make for a good review.

I'm unsure if I should be surprised that no-one has mentioned this, but take a look at The Devil's Backbone. [http://www.imdb.com/title/tt0256009/] It's the first film made by Guillermo del Toro (who did Pan's Labyrinth and the Hellboy movies) so it's got some supernatural themes, but it doesn't have the crazy awesome effects he's usually known for. The supernatural bits aren't really the most disturbing part of the film though.

I've heard terrible terrible things about Salo, so that could be right up your alley. You've already got my first recommendation, A Serbian Film, lined up.
I just watched SALO last night and it is horribly disgusting. I mean the premise of people being kidnapped as sex slaves didn't disturb me much at first, but when I saw everyone at the dinnertable digging in from a bowl full of SHIT and actually eating and enjoying it I fell sick to my stomach and had to take a 5 minute break. So whatever you probably heard was most likely right.

I'll second Come and See (1985), one of the most disturbing (and frank) portrayals of World War 2 ever. It's a psychological horror that's made even more horrifying because you know that unlike ghosts or torture cults or mad scientists, the events depicted actually occurred in real life pretty much exactly how they are shown.

Oldboy (2003) is pretty violent and fucked up in parts. They even have the main actor eat a live octopus, on-screen, for realsies. Three times (he kept nearly choking because the octopi kept twitching and wriggling too much as they were eaten alive). One of the best thrillers ever, though.

Super (2010) also kinda disturbed me. I thought it was going to be a light quirky comedy about that weird guy from The Office trying to be a superhero, along with Ellen Page as his manic pixie sidekick. But it is actually dark as all hell. Darker than Kick-Ass.

I once held a movie night with friends, in which we watched two disturbing films after one another. The first was Natural Born Killers. Great movie, very disturbing. The second was Irréversible. Let's just say that movie was very, very, very disturbing. If you can watch the scene where Monica Bellucci is being raped and beaten without flinching, you've lost all sense of humanity.

What makes both movies so disturbing is that they eschew traditional narrative form, which adds a huge punch to the subject matter.

Not sure how disturbing it is, but Confessions. It's a fairly twisted revenge story.

Although some less disturbing entries on there, I think it's pretty comprehensive. The top 5 especially.

The Woman In Black was really, REALLY unsettling for me. I'm not the guy to scare easily, but that movie kept me on full tension throughout the majority of it's 92 minutes playtime.

I had a massive nightmare the night after. But heh, I really liked that movie.

Try Human Centipede 2, it is more fucked up then the first, with Tom Six even saying it makes the first one look like My Little Pony, (But I'm terrified of 3rd one as Six said it will make the 2nd look like a Disney movie)

I decided to watch all of the most disturbing films a while back......it was a bit dull to be honest, merely waiting for the next uuuuurghh moment!

Anyway, some recommendations

Guinea Pig 2: Flower of Flesh and Blood
Men Behind the Sun
Antichrist
Cannibal Holocaust

That should keep you going for a few hours!

No mentions of Ichi the Killer?

Yeah, this is one of those extraordinarily rare instances where I can actually recommend Ichi the Killer.

Subconscious Cruelty. Haven't seen it myself and seriously, I don't plan to.

There's a delightful little trailer for it on youtube, but linking it here seems inappropriate.
