Interesting points. I guess it's mostly a question of what you're used to: I'd already mostly got the hang of the timing of the old "border of life and death" mechanic, so the added visual cues have never been of much use to me, and the fact that last spells use two bombs at once doesn't seem worth the increased damage.
What you say about the versatility of the teams also made me realise that I've only really been considering bosses. In terms of the actual stages, IN's 4 & 5 were relatively mild; (especially given how nightmarish PCB's 4 was) so much so that the advantage of having two dramatically different shot types was something that never really occurred to me- I tend to keep the shift key held down for pretty much the entire duration of the boss fights, and they were what I was really having to focus on (pun not intended). Well, that and I've been almost exclusively playing as Team Scarlet: Remilia's incredibly useful familiar combined with her high focused speed means that the only reason I ever switch to Sakuya is to collect the prizes.

As for Keine, the fact that she has a wider array of tricks up her sleeve (like aimed clusters of oversized projectiles and criss-crossing patterns of lasers backed up by waves of bullets) in addition to non-spellcard patterns that arguably rival Meiling and Alice's last couple of attacks makes it seem like she is at least intended to be harder. It certainly worked on me, but then I could be in the minority for all I know.

Just today I fought Ornstein and Smough in Dark Souls.
Beat them on my first try as a slow, bulky knight, by myself, melee only.
NEVER AGAIN!

FF13 one boss i had so much trouble with and then i eventually beat said boss and quit the game cause i was sick of Linear

Completing IWBTG on Very Hard or finishing ADOM with an ultra ending. Not sure which is worse, probably the latter.

Dragon Age Origins, Golems of Amgarrak.
Defeating the Harvester on Nightmare difficulty. End of story.
Took me something of 4 hours and quite a few controller-hurling moments.

Dragon Age origins decided to fuck me over one day. It ballsed up and managed to set itself to a higher difficulty than I had set it to. It took me 6 freaking hours to complete the part in the Arl of redcliffe quest where the undead attack the town. Seriously, it just wouldn't end. It was nightmarish.

oh man i feel your pain, although i at least had some awesome pyro shiz. i had to have died on them about 40 times, just didnt have humanity. then managed to score some and some awesome guy helped me beat them. HARDEST. BOSS's. EVER

I'm gonna have to say the final area of cave story. I've played and beaten super meat boy, and the last part of cave story.... that shit is fucking hard.

This. Mostly this. I beat the Elite Four but lost half of my team to do so. Not happy in the least until after I finally beat them.

oh so many to choose from...

fallout new vegas- taking on all the deathclaws at quarry junction with a well placed c4 I had started shooting at all of them and wasn't paying attention to my rounds for the gobi campaign sniper rifle.... damn

left for dead- crowned on accident while running past the witch

dead space 2- fffffuuuuuuu so have you ever had those friends who when you fall asleep don't save and turn off your game they put you in the worst spot imaginable...? sooo no health all the health kits have been used up or just gone right past a spot where i couldn't turn back and go shop so i had to go forward with no ammo lets just say i ran like a coward.

any game where i forgot to heal before a major boss fight but thats more stupity on my part then anything else.

reach- legendary mode with elites coming after me threw down a drop shield and chucked a grenade in before walking out must say thats an interesting way to take down three elites at once. oh also if they came to hear me beg.

It took me something along the lines of two hours to finish it. That might not be long at first, but remember that I retried a level that only takes a literal minute to beat 120 times before finally getting the achievement.

I also tried Black Ops on Veteran. EEEEKKKK!!!! Treyarch is the devil when it comes to difficulties. I never did beat it, though.
My problem with any FPS on high difficulty is this:

You can't dodge a BULLET.
No matter how insane you are, you'll never ever EVER beat a FPS without getting hit. It's impossible. Skill can only do so much, like in Mario Kart.
I'd know. Halo 1-3 on Legendary was pretty bad.

Anyway, It's hard to say what I've done that was the toughest. On Meteos, I managed to get max score on a planet by keeping a single rocket in the air, covering the whole screen for almost 2 hours. On the Metroid Prime Hunters demo, I managed to get INSANE scores that were so high they went completely off the screen. (you got multipliers as you got kills without missing a single shot.) My aim was legendary in that game, and I continued the legacy online in the full game when I reached nearly-max rank (believe me, it's almost impossible with how it's set up) and managed to pull off headshots across the map while accounting for a half-second of lag. I've also gotten some perfect scores in Daigasso! Band Bros. Each one took hours of re-playing the part, because you have to hit every single note perfectly on-time and the game is really strict about timing the rhythm perfectly to get Best on each note. I've also made it to the end of Cave Story on Wiiware on Hard mode. Haven't beaten it yet because it's insanely hard, but I made it to the last boss in a 4-boss streak of battles a couple times. For the uninitiated, you don't get health upgrades or any unnecessary items on hard mode (aside from the single healing item and a health boost right near the end of the game) so for most of the game you die in one hit from almost everything.

I've done a lot of other insane stuff in games, but this is the most recent stuff that immediately comes to mind. I'm a real masochist, the kind of oldschool hardcore gamer who loves a great challenge.
