I'll keep going with the MW2 trend and say the beginning and end of "Of Their Own Accord". The one that starts in a dark, quiet bunker where you walk past all the injured soldiers and meet up with your squad, then head up the stairs to see all the burning D.C. landmark with artillery and planes flying around everywhere, accompanied by Hans Zimmer's epic soundtrack. And then ends with you in a helicopter dodging dozens of RPGs and knowing you'll never make it out, spraying your minigun trying to take out anything you can before you go down. Also accompanied by Hans Zimmer's epic soundtrack.

In Armored Core For Answer when taking on Cabracan the Arms Fort. You destroy the engine easily and then about a bajillion mini-mechs are released. All of which you have to kill. It's especially badass by the end when you're on a backup pistol and all your other weapons are just gone.

Dragon age origins, walking around while I was level 10 looking for the ashes of andastre on my first play through.... Oooooh what's that bell doing here? I'll ring it.... Holy fuck a fucking high dragon!!!!!

A lesser moment, when I first encountered a ARGUS robot in Vanquish. I'd killed it and turned around to head to the door. The door didn't open then I turned around and saw the formerly 4 legged tank had turned into something that more resembled Megatron. It was awesome. Also the final boss when they take the limiters off.

Others are, Batman Arkham Asylum third scarecrow section.
Portal 2 when your escaping.
The first time I played Halo 3 and did the last level, escaping from an exploding halo was rediculously action packed.

"Holy shit" can be a little ambiguous, but I can think of two examples:

Positive: The primary weapon of the Progenitor dreadnought in Homeworld 2. A really nice piece of FX work and that charging & firing sound is just too delicious.

Negative: 'The End of the Plot' cut-scene from the PSP port of Valkyrie Profile. I never imagined something from a game would keep me awake at night.

Bioshock's "Would you Kindly" section was quite mind blowing in terms of twists
but...
Metal Gear Solid 2 inside Arsenal Gear with the Colonel yelling gibberish at you while you run around naked and helpless. Classic chaotic scene.

But my most recent holy shit moment was while playing Limbo, that game has a speed of like 3 hsm/m (holy shit moments per minute)

...the Baron's lawyers fail to win the child labour debate in the zeppelin and so his backup plan is used... Consisting of ejecting the contents of hundreds of fish tanks out onto the top of the zeppelin, causing thousands of pelicans to swarm down on the fish and cause the zeppelin to start to fall out of the sky. If that's not a holy shit moment, I don't know what is...

Arkham Asylum has one of my favorites. If you've played, you know I'm talking about the Scarecrow scenes. At first, it was just screwing with Batman, the character. Then, I think the third time, the Scarecrow starts with you, the player. I won't spoil it.

I agree with the people saying the nuke in COD4. Otherwise I loved the first time I encountered a monster in Amnesia and just ran the fuck away and hid in a cupboard and cried. Also the flying colossus in SotC.

In a more positive note, the end of Hitman 2 where you get to your garden shed. That might be my favorite end for a game ever.

Meeting the Flood for the first time in Halo Combat Evolved. Imagine if some scientist today accidentally released the equivalent of the bubonic plague on the world, and realized what he/she'd done a second too late for him/her to stop it. Sends chills up my spine.
