I guess it depends on the sort of build you have... Here are some tips I use for him, but I've only killed him twice.

On his first form (The Fallen?) push a bomb at it when it splits into 2 for an easy hit and a good amount of damage.

In the second form, by sticking to the side of him he will only fire out the blood balls, which I think are easier to dodge than the lasers, but you need to switch sides quite often as he closes in towards you.

I don't really have anything for the third stage, just keep moving, but that's kind of obvious :/

I'm sure you will be able to find better tips if you want to search for them, but I found those things useful.

On a side note I always get Dr Fetus when I play as ???(The Dead baby), it makes me laugh and scream at the same time.

ot: Already mentioned but I vote for the Witcher 2 first boss fight, made me feel lame for looking at a video of how to do it, I got stumped at the end where you have to go up on some rubble to kill the damn thing.

I'm gonna say every fucking level ever on Touhou, Lunatic difficulty... Seriously...

She isn't even the "real" boss of the game...

this game created the genre "Bullet Hell"... And you can see why...

I was gonna say this exact thing. The pacing and puzzle elements really felt strange. I have heard that there is a mod you can download that allows you to skip that part. I've never tried it, but not having that silly side bar would definitely help the mage level.

Resident Evil 4 when you have to fight Krauser on professional. 34 deaths, 150,000 on a new magnum, and 2 and a half hours later I finally beat him.

I despise sewer levels in almost any given game. Surprisingly I enjoyed the claustrophobic sewers in Dead Island; but I am an absolute fanboy when it comes to that game, so mine is a very biased opinion.
The most annoying level I have ever played in a game, in recent memory, would be the mission where you get Sacha in Metro 2033. I just hated fighting those lurkers when you're forced to move so slowly when the kid is on your back. It was cool at first, but it got aggravating when the lurkers just kept coming, one after the other, for minutes on end. It wasn't even hard it just didn't know when to let up.

Homeworld. The 'holy shit, that's a lot of ion frigates' level. We ended up using cloaked Salavage Corvetes. Catured every one of them.

I think the first time I played Golden Sun the first battle with Saturos took me 2 weeks to finally get through.

Also, this one level in the NES Ninja Gaiden where these GODDAMN MOTHERFUCKING EAGLES keep swooping down and knocking you into pits while you're trying to jump over. Seriously, you know what the biggest threat to humanity is, according to Ninja Gaiden? Not the evil god that's been sealed away, it's EAGLES. I must have died like a billion fucking times to those motherfuckers. The end boss, on the other hand, turned out to be a pushover (By Ninja Gaiden standards, he still killed me like 12 times) once you'd learned all the jumping techniques required to deal with the god damn birds.

Rayman had levels where the top of the level and the bottom were spikes, you had to use the hair helicopter to travel up and over them (like copter, but with story). It pissed me off muchos badly.

The rooftop section in Dark Messiah where you chase the goblin. It may have been bearable if the movement wasn't so godam clunky.

I have quite a lot I'd regard largely as bad, so here's a couple;

Call of Duty: Black Ops - the one where you're defending Khe Sanh from ungainly numbers of NVA troops; half the level you're being shot at by approximately half the enemies in the level from roughly 270 degrees around you, meaning you survive better if you stay down, which results in ridiculous grenade spam, which results in death. Then there's that bit where you have to attack downhill into overwhelming numbers and I found you pretty much have to do it single-handedly because the AI are useless and don't support you.

I also hated that level in Battlefield: Bad Company 2 where you have to defend the satellite from spawning enemies who fire at it immediately, meaning you have to rehearse where they will spawn in order to have any chance of success. Then a I-kid-you-not, an Mi-24 appears and guns it to pieces in seconds, unless you can score a lucky hit on it with unguided rockets... After that, it gets slightly better, save the stupid "You must be in a house or freeze to death!" mechanic. Suffice to say, I gave up and eventually a very persistent friend got me through that level; never done it since. Don't ever intend to.

The first part of a game that comes to my mind is either the maze near the end of Ninja Gaiden Sigma with the zombie ghost fish (why does that remind me of Full Life Consequences?).

That or the dungeon in the original Phantasy Star; the one with the guy who tells you to go left at the start which leads you straight to a hole that has no way out (unless you have the flute or learned the exit spell with MP to spare), trolling before the internet.

The giant wind slashing golem boss in Sen Fortress in Dark Souls. That boss has made me rage delete many characters alone. Then there is trying to get around in Blighttown; leaving it and going to it. Don't get me started on invading characters. The depths made me so paranoid that I was too afraid to have my char in places with high ceilings. Then there is FUCKING GOLEM throwing giant bombs at me while i try to kill the golem boss.

I think I'll stop here or I'll be stuck here typing forever.

Firstly kill the giant that throws bombs at you and then confront the boss. Also if you are on human mode, you can summon Knight Solaire to help you. His sign is in a room close to the area of the boss.

I guess it'll be 5x-20 & 7-20 in Super Meat Boy. Well, any level of Dark Rapture and Light & Dark Cotton Alley can make you shit brix, but these 2 I hate the most.. Also I'd pick 18-19 chambers of Portal Prelude - noy for difficulty, but for trolling the player.
Honorable mention goes to Dwarf Fortress and Binding of Isaac with ???.
