First of all, I love being scared. Not spooks but truly being moved emotionally. The game that has achieved that the most for me is Silent Hill 2, I got into the story so much that it was the story that got into me and scared me. The game itself is not that scary actually, but when you add up the story, twisted monsters and atmosphere it's a terrifying experience. For those who have not played it, the twist in the story is the best (although not the most original, but effective in its delivery). Awsome game.

When I was little it was the Redeads from Loz:OoT.
Now I would say there isnt much that scares me.

In Doom (cant remember which), the end boss, the sound effects of the hooves clomping on the ground. I'd run for hours just away from the boss, afraid to fight him.
Or again in Doom, invisible pigs, id just have to stop playing it scared me that much.

The Black Knight from Fire Emblem 9 (not so much 10, for reasons that should be apparent to those who've actually played the game). Especially in that level when he comes out of the tavern and chases after your guys. He's a walking turn limit!

Both scared me so bad as a kid I didn't play OoT for years. Even today I don't feel safe til I've got the Sun's Song (and remember what you had to sneak past to get to it!).

For some reason, the bandits in the Paper Mario series always scare me as well (they come at you so fast, especially in the Dry Dry Desert - good for creating a 'dangerous desert' mood, bad for exploring the awesome place). It's kind of cool that you can talk to some of them in the second game, but I always keep my hammer at the ready when around them....

Voldo doesn't scare me - he just creeps me the hell out (have you seen his Xenomorph costume? Seriously fucked up, that one is). He's fun to beat the living crap out of, though.

The alma from fear 2 scares me a bit, but not that much.Pyramid head is kinda creepy, but if you cross billy with the joker you get......The dude from condemned 2 that wears some sort of mask. Also, who doesn't find G-man scary?

Those poison headcrabs in Half-Life 2 still scare the living hell out of me. Whoever came up with the idea for those bloody things needs to get an ass kicking.

Saren Arterius from Mass Effect was another one. Not really because of his ruthless methods or even his appearance, but what he had become. His blunt and cold-blooded methods aside, Saren had been a protector of the galaxy willing to fight for what he believed in no matter what the cost. He was manipulated and warped to such an extent that he believed what he was doing was right, even when his actions betrayed his allegiances and very belief system. Having one's views twisted to that much of an extent was just disturbing...

O dear god, the Yao Guai in Fallout 3 scare the crap out of me. A lot of the time I wont notice the thing until it's right next to me, and it'll roar at full volume. I nearly soil myself.

Sander Cohen would definitely be on my list. More subtle version of scary, but he was surely creepy.

Chryssalids from UFO. Scariest. Enemy. Ever. Also the zombies that they turn your soldiers into, because you know they're gonna turn into chryssalids.

Oh man, the Redeads take the cake on this one. I was scared crapless by them the first time I heard their characteristic shriek. They actually stopped me from playing through the adult part of OoT when I saw them in the Market. I assumed that all of Hyrule was now full of them, so I freaked out and avoided the adult Link portion for a while. Nowadays, I'm brave enough to fight them, but they still unnerve me a little. Every time I enter a new room in the Shadow Temple, I play the Sun's Song just to be safe.

And nothing else in gaming has scared me ever since.

it's not so much a game character as much as an animated character from a film that later got worked into a game.

Oogie Boogie from Nightmare Before Christmas...
used to scared the shit out of me.
then later in my life in Kingdom Hearts 1 i got the chance to square off against him, i consider it a fear conquered.
