Okay, first of all, by "overpowered" I mean "stupidly powerful", not "stupidly powerful, but with a major drawback". For example, Limit Breaks in a Final Fantasy game are powerful by design, but they don't count as overpowered here because, well, you have to fulfill certain conditions in order to use them first. However, if it's possible to fulfill those conditions quickly and easily, or the Limit Break in question is disproportionatly powerful to the conditions, THEN it counts as OP here.

Speaking of which, here's my entry: Judgement Triad, from Kingdom Hearts Re:Coded. It's an ability that can be obtained by combining a couple of items from the average item shop, but you wouldn't be able to tell that from how stupidly broken it is. Basically, when activated, Sora throws his Keyblade at the enemy, which splits into three and homes in towards the enemy... and does so much damage, it's not even funny.

How much damage, you may ask? Well, I recently did an experiment. I recently battled the final boss of the game using only Zone of Ruin, arguably the most powerful Limit Break in the game (I used an Action Replay to obtain the attack faster). On the highest difficulty with max stats, it took about 3-4 clean hits with Zone of Ruin to take the final boss down (it only had a small sliver of HP after the third.) That fight took about a minute.

I then engaged the final boss again; this time I'd only use Judgement Triad. Every other variable in the fight was untouched. Ten seconds and two uses of Judgement Triad later, I had won the fight. When an ordinary ability does more damage than the most powerful Limit Break (And oh yeah, is much easier to hit with and can be used FOUR TIMES IN A GODDAMN ROW), then you know you have some balance issues on your hand, to put it lightly.

Think you can top that? What do YOU think is the most OP attack in a video game?

EDIT: Don't forget to say what game your entry is from; you can't guarantee everybody else here has played it. Also, Limit Break-type attacks DO count if it's either really easy to obtain the attack, or the attack is disproportionately powerful to the conditions needed to get it.

Zero Shift - fold space between you and your target so that you can instantly appear behind them and flay em. Fun at first but gets old real fast.

Bone Club as a Cubone in Pokemon Mystery Dungeon.

Suddenly the difficulty goes way down, especially if you stockpile on elixirs and oran berries. After that the only one you have to worry about is your partner dying accidentally.

How about that damn attack that Ares had in God of War? You know, the one where he slams the hammer down, breaking your block and stunning you, and then follows up with an immediate scythe sweep? The one that was almost impossible to dodge? Yeah, that was a one hit hit on God Mode.

Probably Selphie's final limit break.
It kills everything, no exception. Even the final boss of the game can be one-shotted by it.
Here's a video of it.

Ah crap, I forgot it can't be a limit break.
I still love it though. It's crazy insane.

Fully upgraded Vanguard with only a pistol. Nova uses your shield's energy as a weapon and doesn't need time to recharge, yet biotic charge refills your shields

Nova Smash, Charge, Nova Smash, Charge.

You're practically invincible for about 70% of the game.

Well its probably not the most powerful, but the RYNO in R&C going commando was essentially a OHK on all non-boss enemies, had 100 rockets and could fire super fast. Not to mention that ammo was very liberal and if you didn't spend the whole time firing the damn thing you would almost never run out.

Edit: Almost forgot all rockets were also homing.

The Prothean Particle Beam in ME 3 is ridiculously overpowered. Slap on a special ammo type and it becomes an "elemental" laser. I find it most effective to turn it into a freeze ray. :3

Oh, and just for an odd-ball reference. Maxing out your magic in Fable 3 and using dual casting Wind and Spectral Swords (or whatever). Insta-kill most enemies around you while keeping the rest stun-locked? Yeah...that's pretty overpowered. :p

SOTN: Alucard Shield + Shield Rod. crissagrim. duplicator + X. those were all game breakingly powerful.

vandal hearts II: coffin + L-mach armor. it was an srpg, but the coffin weapon lets you attack anything on the map from anywhere, and the L-mach armor let you move anywhere on the map in one move. that's a game breaker combo.

Disgaea... <.< that's all i have to say about that.

The Red Shell armor in Super Mario RPG. Slap that baby on Peach and watch as even Zeromus has trouble taking her out.

Prototype wasn't a very good game. One of the reasons why, in my opinion, was that you could beat it really early on using the whip fist. If you were somehow still having problems, then about halfway through the game you got the armor upgrade as part of the story. So now not only good you one-shot helicopters hovering over Times Square from Queens, you could also walk straight up to a tank, take of the top, and hijack it without the possibility of taking any damage.
The final boss is basically this thing that used to be a girl, and is supposed to be really tough to damage. After all, she has a very small weak point, and there are many things standing between you and her. I only realized this later when hearing someone else talk about it, as well as the fact that you're apparently not supposed to climb up the building she's right next to, equip the armor and either the arm blade or the giant fists, and use a combination of momentum and already strong attacks to kill her in about three hits. That fight against a single hunter took longer, and by the end you were slaughtering six or seven of those a day.
