I lost the crew (with the exception of Doctor Chakwas - didn't know the consequences of not leaving straight away) on my first playthrough but no squadmates, and nobody at all on subsequent playthroughs.

Some of the personnel selections can be a little counter-intuitive at first - the biotic and tech ones are pretty obvious, but when it comes to escorting people back it doesn't matter who you choose as long as they're loyal and when it comes to leading the diversion team it's leadership qualities that count so you need to choose someone used to taking responsibility for others (Garrus, Miranda, I think Jacob works too?). And for holding the line at the end you need to leave your tanks behind (Grunt, Zaeed, Garrus are the three big-ticket ones from memory).

FWIW you should be able to reload prior to the suicide mission, make different choices and ensure a lot more people survive for the game you import to ME3 (if that's what you plan on doing).

For some reason mordin seemed to die in my game and my friends game, like offhandedly
even if he we loyal to you
i made Jacob(who was also loyal) lead a diversion team because his profile said he was a leader and he got mordin killed

Paragon Infiltrator fem-shep got everyone through, cos she's a boss.

Renegade Vanguard male-shep pushed Jack out a window, cos you really don't wanna wake up with her standing over your bed, thats never gonna end well.

First time through, Mordin and Miranda. Only one of which I mourned, and I'll leave it to you to guess.

Second time through, Jack and Legion. Jack, because of that damned dialogue with Miranda after the Loyalty Mission, and Legion because I never did his Loyalty.

Third time? No one. Same goes for playthroughs four to bajillion.

First time round, farted around doing loyalty missions and stuff, then found out the hard way that the 'rescue' bit is timed. Sorry Kelly, the funny engineers and everyone else. Everyone else survived though.

Second time round, everyone survives (although i flogged Legion off)

Third or fourth time round, i start trying for a TPK. I know I lost Legion (shot in the face since I didn't bother w/ his loyalty mission), Thane (I put Miranda in charge of field generation), Jack (fire team leader), can't remember who on escort (I think it was jacob - again no bothering w/ loyalty), Miranda (I think) and (i think) Mordin to the final boss, and Garus (possibly someone else as well - i can't remember for sure) to the hold the line (I didn't have any DLC charries at the time). That game's gonna be interesting when ME3 rolls around, if i'm remembering right

First of all loyalty missions and ship upgrades are most important.

for the tech part send either tali or legion

for the 2nd squasd leader garris or miranda will do fine

for the escort part anyone loyal will do, I always used grunt

For the shield part the asari and jack will do and I think miranda would work but not 100% sure

5 playthrough's never lost a single person.

If I sound nerdy I apologize, I love ME2.

Everyone in ME2 has a score, ranking how strong they are and how likely they are to live. Mordin has the lowest score since he's a salarian scientist and probably someone like Grunt has the highest score. The score isn't determined by loyalty, loyalty determines focus, and focus can get anyone on your team killed if they dont have it. If you send a strong individual on a mission, who is loyal and is right for the job then hey thats great. But what people need to keep in mind is that group firefights also have a set score and if its not met then someone with a low score dies. If, on the final decision, you send Grunt with the survivors. Take people like Garrus and Jack with you, and send someone lame to lead the fire team, there will be alot of dead people since the fire teams score will be very low since there arn't alot of strong people to help them. Tali died in your game because her score is low since shes a sickly Quarian. Jack died because she lacked focus

SO, my answer to you, Mordin died randomly in the fire team fight because I sent all the strong people I had away to do other things or fight with me. This is why Miranda and Thane are on the front cover of the ME2 box art. Taking them insures no deaths in the fire team since their scores are average, and they wont die with you during the fall if they're loyal to you. Miranda died in my first play-through due to me picking Jack and not being a super renegade/paragon.

When it came to the final mission I originally lost Mordin. But there was no way I was gonna let that slide so I replayed the mission and lost nobody. I wouldn't care to much if I lost Jacob or Jack. But the hell if they think I'm gonna play Mass Effect 3 without any Mordin Solus in it.

Originally Mordin died for me, despite doing every loyalty quest, having all the ship upgrades and going after the collectors in time to save the crew. I guess I messed up on choosing an ideal candidate for each job. He randomly died right at the end, probably because he was too weak to defend the door, I think I had Garrus with me, so I swapped the two, took Mordin with me and saved everyone. Not sure how it'll go in the second one or if they'll offer as many chances to die (probably) but I'm not sure if I'll go ridiculously far out of the way to save everyone, seeing people die could be cool and raise the stakes but they'll probably make saving everyone an achievement again, in which case I'll probably do it.

First time through without looking into how it all worked I think I lost Tali and Mordin. Tali didn't bother me but I was pissed about Mordin. He was fantastic. Second time...everyone lived cause I figured it out. Third time....I don't recall. I know I lost quite a few people cause I didn't give two shit about the moral system so it sorta jacked with things. Looking forward to ME3. Shame about it being all Origin and stuff though. Le sigh.

Everyone died except for Miranda. That game's ending was a blood bath for me.

The trick with Mordin is to have him escort the non-combatants back to the Normandy.

Anyways, I've played through twice. As I lean far more towards the Paragon options than the Renegade ones, this of course means that by the time the loyalty missions popped up I had a silver tongue and could work my way out of anything. Ergo, everyone was loyal when I took on the final mission (Party conflicts? I laugh at them. Ha! Haha!), and I made it out without a single casualty in both playthroughs. Which was good, because I am very protective of my allied NPCs.

Miranda and Legion I didn't have a good relationship with (not enough Paragon), so they both died holding the line, which I had expected (though I was sad about Legion). Tali also died by headshot near the beginning after she did her hacking part, even though I had a good relationship with her.

I had all of the upgrades, and I already said that Legion and Miranda weren't loyal. Here were my choices for support:

Hacker: Tali
1st Squad Leader: Samara
Deaths: Tali

Biotic: Jack
2nd Squad Leader: Garrus
Sent back with crew: Grunt

Party for final boss: Samara and Garrus
Died at the line: Miranda and Legion

Problem highlighted above - even though Tali was loyal, whoever you send through the tunnel will die if you don't have a good enough squad leader. Switch Samara (who's an excellent fighter, but not so great a leader apparently) for either Garrus, Miranda (maybe not her if she's not loyal) or Jacob and your tech expert should live once they exit the tunnel. Since fighting Collectors calls for biotics a lot more than tech skills, I just have Garrus be the other squad leader pretty much every time.

Also, it's best to keep Grunt around to "hold the line" at the end since he's, well, boss at holding lines. Your weakest loyal squad member is usually the best one to send back with the crew.

I dared not let any of my team die. Sure, I cheated to get there, and I don't have the time or accepting girlfriend to let me have another, fairer go at it, but I'm going into Mass Effect 3 with EVERYONE alive!

...except, y'know, Kaidan.
