Milla's secret chamber is one of the best, but my favorite is from Silent hill 2, really close to the end, when the staircase is in flames. Angela starts walking up the staircase, and James says "It's hot as hell in here." she stops, looks back, and says, "You see it too. For me, it's always like this." and then turns around and slowly walks into the fire. Not only is it incredibly freaky and creepy and depressing and tragic, the music is perfect for it. Way more disturbing than the rape scene, at least for me. But even more disturbing then that is the abortion-reconception-birth scene in Silent Hill 3. Never has baby-eating been so vile.

Here is one that I dont think anyone else will mention.

SPOILER WARNING FOR MASS EFFECT 1 DLC!!!!

In the Mass Effect downloadable content Bring Down the Sky one of the objectives is to get the leader of a terrorist group. When you finally get to him you are presented with a choice of let the terrorist go to save the hostages or let the hostages die to get the terrorist. I went after the terrorist leader and condenmed the hostages to death. The parts that made this a real emotional hit to the gut are
1) that you are in communication with the hostages before they get captured so you begin to know the woman you are talking to which humanizes the character and
2) When I was exploring the map looking for stuff I accidentally stumbled across the remains of three charred corpses. This made me feel a lot of remorse for my actions and explaining the situation to her friend was also bad.

Something about seeing the mangled remains of the innocent people who died from my actions really hit close to home. Even though it was a video game it still emotionally felt like I did something horrible and I had to tell myself how it was the right decision based upon how much damage the enemy leader can do if he escaped. To me it was very emotional stuff.

Many things in games don't bother me. I've been desensitized. There's this one part on the level before the finale on Blood Harvest on Left 4 Dead where there's about half of a guy on the ground and blood is covering the room.
Go to about 4 minutes of that to see a little bit of what I was talking about.

Also, I had never chainsawed anyone in Gears of War one before and the first time I saw the blood shoot out it mildy disturbed me. I was used to the juicy fake looking blood of Gears of War 2 but in the first game it shot out like spray paint or something.

What about that spider splicer in the beginning who slammed the guy against the bathysphere and gutted him right in front of you? That was pretty messed up.

Didn't make me jump. Unless by jump you mean start tearing up and I swear I almost broke down and cried. I loved that guy.

the first time i saw a deathclaw in fallout 3

this may sound wimpy but also the fire ant, damn that actually legitly scared me!
Gears of War 1 theres a spot when your going to marcuses house, its kinda a secret place you walk into like a bathroom theres some ammo, but them you have the option of kicking open the stall door, theres half a body hanging by its legs above the toilet,blood every where and it does that startling music that was weird i had to sit for a secound before i could continue!

Oh it doesn't look like much, but the further you go the more the game does to mess with your head and creep you out. There wasn't any particular part that was disturbing, but the cumulative effect left my nerves so frayed by the end that I couldn't stop laughing (in that nervous way)

The Path. Once you meet a wolf you'll never be the same again. Ginger, Ruby, Robin and Rose are the worst; though they're all quite traumatic. Play the game, and if you don't feel guilty, sad, unnerved or down right scared, then you have a heart of stone.

I think there's a very big difference between "scared/disturbed" and "startled". Like in resident evil 4 when the burning guy bursts out of the fridge: not scary, but definitely startling.
The first game that has genuinly spooked me was Clock Tower: First Fear on super Nintendo. I don't mind zombies, aliens, werewolves, vampires,anything like that. Clock Tower scared me because it was pure psychological terror: the feeling of helplessness. Clock Tower 3 on PS2 was weak, simply because you could actually fight back. (well, OK, the whole killing the little girl thing was pretty freaky).

Condemned scared me a lot, but the only thing I can remember disturbing me was the locker room (the Tibbits scene). That had me looking over my shoulder for quite a while.

When I played Ocarina of Time as a kid, walking out into the marketplace after becoming an adult and seeing Redeads everywhere scared me a bit. When I heard their scream and had one jump on me and start sucking the life out of me, I was immediately fucked in the head and had to turn the system off immediately. I had nightmares. It was horrible.

The first time I played The original Doom on nightmare difficulty. Something about not being able to kill my enemy's for good and having them rise up again and again to get me freaked my little 9 year old mind right the hell out.

Also... Playing Zombies on Halo 2 when your the last human left and your high as fuk. That'll get your blood pumping.

ummm im gonna say... when i stopped to think about the concept of pokemon. you are an 11 or 12 year old (i cant remember which) that goes off on their own with little money to fight monsters and you try to take down the mob... wtf not to mention the fact that everyone thinks its ok to kidnapp monsters and make them barbarically fight (but take turns) and the fact that in the movies ash almost dies in all of them... idk about the new ones cuse im over the age of 12 but im assuming they still do the same thing and no one calls him on it. and this obvious pedofile profesor oak just encourages them to go off and be on their own. thats my rant
