Crack The E Nigma on Batman AA without looking stuff up.
I am proud to say that I got that Achievement, and only looked up one thing. I say that that one doesn't even count, because I was just looking for the last three chattering Joker teeth.

test of faith: beat mirrors edge without shotting an enemy. wish i'd never tried
I did that on hard difficulty! My first play through o hard I decided not to shoot anyone...it was near impossible at times!
That was the best part about that game. Did it the first playthough on hard, only fired a single bullet (sniper shot, required to take out a vehicle engine) the whole game. It was tough, but certainly not impossible.
Ah well you see, I'd gotten used to shooting them. And I agree it was more fun not shooting them, but the boat level seemed really hard

MGS2 E.Extreme with less than six saves and under two hours. I found it fun to inflict such pain upon myself...

Of recent;
Trials HD: Marathon (20 consecutive challenges no faults)
Blazblue: Leonidas (Stupidly hard end boss, capable of death juggles from the first millisecond)
N+: 50 SP Episodes and 10 MP Episodes, not insanely difficult but enough annoyance

I have quite a few like this, but these are the immediates that spring to mind

Devil May Cry 4 on Son of Sparda difficulty. That battle with Dante was the worst. I only got through it because I trapped him in an infinite combo with the devil bringer with Devil Trigger active.

Don't even get me started on Dante must Die difficulty (Gah, I'm stuck on like the 9th level, and now I'm out of practice. No way man.)

There's also another achievement that I SHOULD have, but I don't, and I don't feel like doing it over again. In Prototype, there's an achievement for finding all 200 location orbs. I used an online walkthrough for the achievement and ended up with 199 out of 200. That made me so sick that I didn't play the game for months.

Anybody mentioning Modern Warfare 2's campaign on veteran has clearly not played World at War's. Grenade hell. CoD3's was very frustrating as well, mostly due to the buggy cover and shoddy hit detection.

Eh, that's not that bad. There were a couple I did look up, but even without a walkthrough, I probably could have figured them out eventually. The main reason they're so easy is because of the maps you can find that pretty much blatantly tell you where the solutions are.

That halo 3 skull on sandbox. Did the whole thing by building a massive tunnel. Took me for fucking ever and I died so many fucking times.
Best bit is, turns out there is a cheat way to do it. Out-bloody-standing!!!!

I'm not sure why people complain about Test of Faith. I got that one on my first playthough. It's easy, considering that you can a) melee, and b) just run away. Bobbleheads in FO3 was also not bad... most were close to locations necessary for other achievements, so I just got them along the way.

The worst achievements ever for me were The Final Instance and The Last Remnant (closely related achievements) from The Last Remnant. They required excessive grinding in a game that was already long and dull, and the bosses were mind-blowingly hard. I keep all of my games- never trash or trade? but I very much considered setting fire to that disc when I was done with it.

Also, Compulsive from Infinite Undiscovery. Truthfully, I never got that one. I should have, but somewhere in the hundreds of items, I must have missed one. Dozens of hours of work for nothing. Goddamnit.

I don't play Square Enix games any more. Guess why.

I think it would be annual on Halo 3, not a hard achievement by itself but try getting it with 2 more people who know what they are doing but a fourth who fucking SUCKS it was the 3 most annoying hours of my life

Attaining all the veteran achievements on Modern Warfare 2. On that difficulty it turned a 5 hour game into a 10 hour one. Trying to complete the last part of contingency in only a minute and a half was the worst.
When I tried that game on veteran it didn't seem too much more difficult. Except for the FUCKING FAVELA LEVEL!
Gods, how I hate that level, hours of frustration and getting nowhere
Yeah I didn't find MW2 that tough even on veteran. Didn't have that much trouble at any point really. Modern Warfare 1 was much more difficult.

OT Anything from Brutal Legend only because the game is so terrible. I never even finished it.

Oh, you mean where you overload the map?
