The Annual achievement in Halo 3. You have to get through the last level on the highest difficulty with 3 other retards constantly taking all the supplies and dying with a skull that forces you the go to the last check point every time someone dies. Additionally you have to complete the warthog run (a race across collapsing platforms) on single person ghosts. Not fucking fun, god damn it!
Oh man, I would do this achievement over and over and over. I did it with 3 close friends, it took us all night, but IT WAS SO MUCH FUN.
Similarly, lots of people on about Brass Balls from BioShock and the same thing in BioShock 2. I found those extremely fun, like the way the game should be played. Same thing with L4D expert.
OT: Burn'em Out Veteran in CoD:WaW. Lots of people in this thread complaining about MW and MW2 veteran. MW1+2 Veteran is a cake walk compared to ANY level in WaW, and Burn'em Out was only about halfway through the campaign. Took me a few hours of pure rage to get. I eventually said screw it to beating that game on vet, too hard.
Also, Aperture Science in Portal, for getting all the gold medals on the advanced challenge things, like least steps, time, etc. It basically requires you never stop moving for the time ones, and pinpoint accuracy, and for steps you basically need to exploit. For portals, those ones aren't too bad with a guide. But I got 9/18 on the achievement, after two pure days of rage, when I did the EASIER challenges. My progress got reset. Never again.

L4D2. Heavy Rain. Sneak past all the Witches. We had one- ONE- AI in the party and I only just managed to ding the achievement. AI spazzed, the last Witch was startled, and my fiance's chances at getting the achievement was screwed.
Ironic part? I'm scared to death of thunderstorms, so we had to keep stopping when the worst parts of the storm hit so I could have panic attacks. Yes, I'm a giant fricking pansy, I know.
Hate to tell you this, but the achievement is simply don't kill any witches, not, don't startle any witches. If the AI startled one, you could have just let it die and walked into the saferoom, and everyone would have still gotten the achievement.
