Water... I just hate it when the enemies can easily outmove you when swimming. Usually some killer fishies or so. Great example would be those electro fishes in Serious Sam or slaughterfish in Skyrim or Oblivion. Other speed maniac monsters probably count on that also as long as the terrain is not open enough for me to kill them from afar before they get too close.
For some reason playing Amnesia was a bit of disappointment to me. Heard it was a scary game. Friend has it. Asked me to play it... I was just running along everywhere with no worries at all while my friend was freaked out behind me just by watching me play XD
And just so you know. I'm really bad with horror movies and hate gore in movies maybe more than anything else. In gaming I don't mind neither (played through Bioshock 1 and 2, haven't tried Dead Space yet, going to get it someday though)

1-shot kills, I hate them so very much. Especially if they are unblockable attacks and/or are very hard to avoid. Example is the Dark Souls bush monster face noming. Get swarmed, get surprise hugged, get nomed, get dead.

Getting swarmed is another thing I hate and do what I can to avoid.

Nope. My real-life phobias (heights, among other things) do not extend to video games. Otherwise I could never play a game like Mirror's Edge or Assassin's Creed or even Saints Row 3.

Chase scenes, that feeling of "WHAT DO I DO, IT'S RIGHT BEHIND ME, WHERE'S THE FUCKING DOOR, OH SHIT, WHAT DO I DO."

Water sections with huge swimming monsters. Eels, sharks, snakes, whatever, they freak me out.

Water, god help us all if there is water. Whether it is almost drowning or being knocked out of the boat in wind waker it scares the hell out of me. The biggest offender being this:http://www.youtube.com/watch?v=68u77N6ghLA&feature=related

My only real in-game phobia is of deep water and shipwrecks, which stems from my real-life phobias.

Another case of hydrophobia here. Ingame swimming still sucks, especially because of the horrific camera angles and poor underwater mechanics. Chronophobia too, to Hell with time limits. I also have a crippling separation phobia involving save points. I don't wanna be away from one for too long.

Silent enemies specifically the mirelurks from fallout 3 I hated having to go into the nuka cola plant because it they'd sneak up and scare the crap out of me especially the glowing ones it was just eerie to see it silently coming up to you and knowing you had to wait till it was right in your face before you could kill it. I forget what vault it was but it was full of them and that was one of the few parts of that game I did a speed run of. Strangely enough I was never worried about the mirelurk kings, odd.

Swarms of large bugs, like Cazadores in New Vegas. What's funny is, the Giant Cazadore doesn't bother me as much. Giantness makes things less scary.

The idea of being stung to death by eight or nine Tarantula Wasps the size of large house cats terrifies me. It would be so slow and painful. Imagine how horrifying it would be as you were dying and you felt them crawling all over you starting to feed/lay eggs/whatever they do to your corpse. I'd go insane as I died...

*Shudder*

The fear that I didn't defeat an enemy in the most efficient way possible. So I would defeat an enemy three or four times, each trying to use weaker and weaker weapons, or hitting the bad guy in the special weak spot, or trying not to miss once.

I can't say I'm really as afraid of it as I should be, since I don't save as often as I should, but I really hate restarting. It bugs the hell out of me especially in RPG when everything's going right: I'm choosing the all the right dialogue options, finding all the right loot and equipment, having all the right encounters and kicking ass in combat. But then I get killed cause I made a careless stupid mistake like I forgetting to check my health level and some low level grunt manages to land a hit. Not only do I lose all my progress but the game never goes as well the second time around. When I know there's gunna be a big mission or there's an encounter with a particularly tough boss, it doesn't bother me cause it's par for the course. Other times: RRRRRRRRRRRRAAAAAAAAAAAAAAAAAAAAAAAAAAGGGGGGGGGGGGGGGGGGGGGEEEEEEEEEEEEE!
Dude, that goes with anything, not just games. I know sometimes I would write a blog entry on some political bullshit, or something, and then lose half of it and need to rewrite it, and it's never as good the second time I write it.

The only things that are remotely similar to 'phobias' in my gaming experience is being a let down for my teammates in a game.

For example, in UT2004 I hate giving valuable weapons (such as the shock rifle or lightning gun) away to enemies, but because I'm conscious of that I end up making that my main focus of the game instead of doing damage to the other team.

In MP games I'm a self-confessed "Teamwork whore", and I hate not contributing/poorly contributing in team games where teamwork is vital/necessary.

I hate bugs, and silence.

Because if it is quiet...then it is always too quiet and you know that shit is about to go down
