Since I have yet to play a "perfect" game, a 10/10 game to me is one that's SO damn good that any faults it has are overlooked because of the overall amazing gaming experience.

That said, my 10/10 games are The Orange Box (yes it counts as one game) Bioshock, the MGS series, Fallout 3, and Pokemon Snap. Yes Pokemon Snap, I can still play it anytime and have a lot of fun with it.

I don't think a game can be perfect, but some games that I think are nearly perfect are:
Half-life 2
Portal
Fallout 3
saints row 2
Battlefield bad company 2
annnnd Time splitters future perfect
Those games are the games closest to perfection in my opinion.

There are quite a few that come near the Perfect 10 for me:

Vagrant Story
Thief 1+2
Planescape Torment
Baldur's Gate 2
Vampire: Bloodlines
Realms of the Haunting
Chrono Cross
Xenogears

and so on. Yeah, I'm a retro.

I don't have very many.

Donkey Kong Country 2 is really up there. Not a perfect 10, but just shy.
Respect awarded

For me I'd have to say Kingdom hearts I and II, GTA San andreas (And I'm not even a fan on sandbox games), portal for obvious, Bioshock. and it wasn't a 10, but it was so Different and amazing and effective..

Those games aren't old enough to be retro......

Star Wars Republic Commando and Legend of Zelda: Ocarina of Time.

I'd probably also add Knights of the Old Republic. I can honestly say that's the most I've ever felt engrossed in a games story.

Amuses me that so many people are rating 10/10 games that they just finished playing yesterday :|

10 out of 10 doesn't mean it's perfect.
Well, that's what a whole fraction means to me. If 10 out of 10 isn't perfect, what do they give perfect games?
Prefect games don't exist, so they don't give them anything... and if one did exist, it'd be treaty like Jesus and spawn its own religion...

None. I have yet to play a perfect game with no flaws. Highest score a game will get from me is a 9/10.

Persona 3 in any incarnation,The characters are great the A.I is actually pretty smart most of the time, the story excellent and the dungeon crawling/Dating sim gameplay works perfectly.

Yoshi's Island for still being fun as hell

Probably Kingdom Hearts 1 and Metal Gear Solid 4. Kingdom hearts was just so much fun, had so much to do and was such a great orginal story. Metal Gear Solid 4 has so many ways you can complete it and the second last chapter and the last fight are probably one and two in single momments in gaming.

To agree with the general consensus of this thread, there is no unanimously "perfect" game, and probably never will be (unless the hivemind takes over). However, with that said, I still consider a few games close to perfection-

Jet Set Radio (AKA Jet Grind Radio) (Dreamcast)
Sonic the Hedgehog 2 (Mega Drive, mostly here for nostalgia)
Pokémon White/Black (Nintendo DS)
Pokémon SoulSilver/HeartGold (Nintendo DS)
The World Ends With You (Nintendo DS)
Final Fantasy IX (Playstation 1)
The Legend of Zelda : Link's Awakening DX (Gameboy Colour)
Persona 4 (Playstation 2)
